Please mail completed entry form with check(s) made payable to “Willow Park Men’s Club”. Entries received after the deadline may be placed on “Standby”. Only entries with full payment will be included in the tournament.

For “Pairing Entry”: Check the box “Pairing Entry” include only member’s first and last names. Pairing members must complete a separately entry form for the tournament and their entry received on or before the deadline.
For “Group Entry” Check the box “Group Entry” include first and last names, plus the information in the group info box. Group entries must include all fees for members of the group.
